Residential Home Health and Residential Hospice is seeking a Home Health RN to provide in-home skilled nursing care in the Danville, PA area. The role offers a first-year bonus opportunity and growth within a nationally recognized leader in home health and hospice.

You will perform assessments, manage medications, and develop care plans while documenting thoroughly in the EMR. A current RN license, active driving ability, and 1 year of RN experience are required.

Qualifications

• Active RN license in the state you work.
• Minimum of 1 year RN experience delivering patient care.
• Current driver’s license and ability to spend ~20% of your day driving to patient homes.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families on disease management and treatment plans.
• Think critically, solve real-time problems, and ensure compliant EMR documentation.
• Work independently, but collaborate with team for comprehensive care.
• Travel to patient homes within the assigned territory.

RN Qualifications
