Job summary

AHN Healthcare@Home is seeking a Registered Nurse (RN) to provide skilled nursing care in patients' homes in Freedom, PA. This role offers autonomy, meaningful patient relationships, and strong career growth, with a $10,000 first-year bonus for achievement milestones.

You’ll travel to patient homes within an assigned territory, collaborate with an interdisciplinary team, and receive ongoing leadership support and a comprehensive benefits package.

Qualifications

• Active Registered Nurse license in the state you work (PA).
• One year of RN experience in home health or similar setting preferred.
• Valid driver’s license and ability to drive to patient homes 20% of workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families on disease management and treatment plans.
• Ensure accurate EMR documentation and travel to patient homes within territory.

RN Qualifications

• Active Registered Nurse, RN license in the state you work

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
