Job summary

Columbia River Home Health in the Tri-Cities area seeks a compassionate Home Health Registered Nurse Case Manager (RN) to lead patient care at home. You will perform intake assessments, create care plans, and coordinate with physicians and families to support disease management and safety.

Qualifications

• Current RN license in Washington State.
• Minimum 2 years nursing experience.
• Home health experience preferred but not required.
• CPR certification.
• Licensed driver with reliable, insured vehicle.
• Strong communication, clinical judgment, and patient-focused care skills.

Responsibilities

• Complete home health intake assessments, medication reconciliation, and care plans.
• Collaborate with physicians, caregivers, and interdisciplinary teams.
• Educate patients, families, and caregivers on disease management, safety, and recovery.
• Maintain accurate clinical documentation and communication with staff.
