Job summary

The Pennant Group, Inc. is seeking a dedicated Home Health Registered Nurse for Thurston County, WA. This full-time RN will perform in-home assessments, develop and implement care plans, and deliver direct patient care under the guidance of physicians and care teams.

Qualifications

• Active RN licensure in the state.
• Experience in home health care.
• Knowledge of OASIS assessments.
• Current BLS certification (AHA).
• Valid driver’s license with insured vehicle.

Responsibilities

• Perform in-home patient assessments.
• Develop care plans under providers’ orders.
• Provide direct patient care per the plan of care.
• Ensure accurate and timely documentation.
• Participate in the QAPI program for quality care.
• Coordinate interdisciplinary home health services.
• Cover weekend and after-hours on-call on a rotating basis.

Key Responsibilities:

• Develop care plans under primary care providers’ orders
• Provide direct patient care per the plan of care and scope of practice
• Ensure accurate and timely documentation
• Participate in QAPI program for top-notch patient care
• Coordinate interdisciplinary home health services
• Cover weekend and after hours on call on a rotating basis

Qualifications:

• Registered Nurse (RN) licensure
• Must have prior Home Health agency experience
• Must have OASIS knowledge
• Current BLS certification from AHA
• Licensed driver with an insured automobile in good working order
