Alpha Home Health & Hospice of King County is expanding its Home Health team and seeks compassionate RNs to provide skilled nursing care in patients' homes across the Seattle/Bellevue area. This full-time position offers flexible scheduling, autonomy, and meaningful impact on patients and families.

Responsibilities include comprehensive assessments, care planning, medication management, wound care, and collaboration with physicians and the care team. Washington RN license required.

Qualifications

• Active RN license in Washington State.
• Current BLS certification.
• Valid driver’s license and insured vehicle.
• Home Health or Hospice experience preferred, but not required.
• Strong clinical, communication, and organizational skills.

• Perform comprehensive patient assessments and ongoing nursing evaluations.
• Develop, implement, and update individualized care plans.
• Provide skilled nursing services including medication management, injections, wound care, and treatments.
• Monitor patient progress and communicate changes in condition to physicians and care team.
• Educate patients and caregivers on diagnoses, medications, treatment plans, and disease management.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Complete accurate EMR documentation.
• Ensure compliance with Medicare, Medicaid, state, and agency regulations.
• Participate in interdisciplinary team meetings and case conferences.
• Promote patient safety, independence, and quality of life.

What You’ll Do

As a Home Health RN Case Manager , you will provide skilled nursing care to patients in their homes while coordinating and managing their overall plan of care.

Key Responsibilities
