Job Overview

The RN Case Manager administers skilled nursing care to patients in their homes, following physician orders and the care plan. Responsibilities include patient, caregiver, and family counseling, communication with healthcare providers, and participation in interdisciplinary assessments. The RN collaborates in developing and revising care plans and supervises LVNs and HHAs.

Qualifications
  • Current RN license in practicing state(s)
  • Minimum two years of experience, preferably in Home Health
  • CPR certification from AHA or ARC
  • Strong communication and interpersonal skills
  • Valid driver's license, auto insurance, reliable transportation
  • On-call availability at least once a month
  • Willingness to travel up to 100%
Educational Requirements
  • High School Diploma or GED; graduate of an approved nursing program
Physical & Working Conditions
  • Work involves indoor office and patient home visits, requiring physical effort including lifting up to 100 pounds, kneeling, reaching, and exposure to environmental elements and biological hazards.
