Responsibilities
  • Supervise and manage patients' care episodes from admission to discharge
  • Coordinate patient care with clinical staff and medical providers
  • Perform home health admission visits, recertifications, and discharges
  • Record, assess, and observe patient progress and symptoms
  • Develop and implement care plans based on assessments
  • Check vital signs, administer medications, give injections, and dress wounds
  • Educate patients in accordance with physician recommendations
  • Document care in patient records following agency, state, and federal guidelines
  • Provide home care post-illness, injury, surgery, or for patients with acute conditions or on pharmaceuticals
Requirements
  • Graduation from an accredited nursing school and a valid RN license registered in Texas
  • OASIS training
  • Valid Driver's License
  • Willingness to travel within the assigned territory
  • At least 1 year of home health nursing experience required
