RN STAR+ LTSS Service Coordinator Level 1 (North Houston)

The Star+Plus (LTSS) Service Coordinator Level 1 is responsible for managing members' cases within the scope of licensure, providing supervision to non-RN clinicians, and developing care plans to meet members' needs, aiming to optimize health care across the continuum.

Job Specifications and Core Competencies
  1. Perform telephonic or face-to-face assessments to evaluate and coordinate members' physical health, behavioral health, social services, and long-term supports.
  2. Identify high-risk members and coordinate care with the healthcare team.
  3. Manage members with chronic conditions and disabilities to ensure cost-effective utilization of benefits.
  4. Gather comprehensive member history to develop personalized care plans.
  5. Establish goals collaboratively with members and their support network, and identify candidates for alternative care or waiver programs.
  6. Develop and oversee access to services, facilitating authorizations and referrals as appropriate.
  7. Collaborate with Medical Directors and interdisciplinary teams on care plans.
  8. Supervise and coordinate work of licensed professionals other than RNs, providing guidance and performance feedback.
  9. Refer members to internal and external resources, and follow up with providers and programs.
  10. Contribute to process improvements and departmental goals.
Qualifications
  • Preferred: North Houston area experience and home health background.
  • Required: Unrestricted RN or NP license in Texas.
  • Preferred: BSN, CCM certification.
  • Experience: 3-5 years in nursing with chronic illness management, 1 year with waiver services, and working with persons with disabilities or chronic conditions.
  • Proficient in Microsoft Office, clinical documentation platforms, and internet use.
  • Must have reliable transportation, valid driver's license, and good driving record.
