Interim HealthCare in Stevens Point, WI is seeking a Home Health Registered Nurse (RN) to join our team in the Stevens Point area. This full-time, day-shift role offers one-on-one patient care and the flexibility to manage your day while staying connected with a supportive medical team.

Qualified candidates will have a Wisconsin RN license, a minimum of 1 year of nursing experience (preferably hospice or home health), CPR certification, and knowledge of home health regulations.

Qualifications

• Graduate of an accredited nursing program and active Wisconsin RN license.
• Minimum of 1 year nursing experience, ideally in hospice or home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing, strong communication and compassion.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility.
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care.
• Examine patients, perform assessments, document progress and report changes to their physician.
• Educate patients on their plan of care, medications, therapy, diet and exercise.
• Coach family members on the patient’s plan of care, medications and home safety.
• Coordinate discharge from services to ensure goals are met.

As a Home Health Registered Nurse, here’s a big-picture view of what you’ll do:
