Job summary

Interim HealthCare Hospice in Hudson, WI is seeking a dedicated Hospice Registered Nurse (RN) for full-time coverage in the Hudson area. Base salary ranges from $80,000 to $90,000 annually, with bonus opportunities and mileage reimbursement for eligible travel.

Join a multidisciplinary team to provide compassionate end-of-life care, educate families, and support patients and their loved ones. Active Wisconsin RN license and a 2-year degree required; CPR certified; 1 year nursing experience

Qualifications

• Graduate of an accredited nursing program.
• Active Wisconsin RN license.
• CPR certification.
• Minimum of 1 year nursing experience, ideally in hospice or home healthcare.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based care to patients with focus on quality of life.
• Work with an interdisciplinary team to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, monitor pain, and administer medications.
• Ensure accessibility of medical equipment and supplies.
• Educate families on prognosis, changes in care plan, and hospice goals.

Job Responsibilities

• Provide home-based care to patients with six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family

Requirements

• Graduate of an accredited nursing program and active RN license in Wisconsin
• Minimum of 1 year of nursing experience, ideally in hospice or home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty, strong communication skills and compassion
