Job summary

Interim HealthCare, the nation’s first home care company, is hiring a Home Health Registered Nurse (RN) to join our Stevens Point, WI team and surrounding area. You will provide one-on-one home-based care and lead a team of LPNs/CNAs in a patient-focused setting.

We offer a $75,000 - $90,000 base salary, bonus opportunities per pay period, mileage reimbursement, PTO/holiday pay, comprehensive medical/dental/vision insurance, 401(k), and ongoing CEU training to grow your career in home health

Qualifications

• Active Wisconsin RN license.
• Minimum 1 year of nursing experience.
• CPR certification.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases at home.
• Oversee LPNs, CNAs and aides providing patient care.
• Examine patients, assess and document progress, report changes to physicians.
• Educate patients and families on plan of care, medications, therapy and home safety.
• Coordinate discharge from services to ensure smooth transition.
