Advantage Home Health Services is seeking a Home Health Registered Nurse (RN) for PRN weekend shifts in Altoona, PA and surrounding areas. You will perform Start of Care assessments, draft care plans, reconcile medications, and ensure accurate orders and documentation.

The role emphasizes patient-centered care, coordination with Case Managers, and clear patient education at home. The ideal candidate holds an active RN license in the state and may be a New Grad with NCLEX eligibility.

Qualificações

• Active RN license in the practicing state.
• Experience with Start of Care assessments and OASIS is preferred.
• New grads eligible to take NCLEX may apply.

Responsabilidades

• Conduct Start of Care assessments and timely complete OASIS.
• Draft care plans, reconcile medications, and verify orders.
• Prepare accurate, orderly documentation and reports for Case Manager.
• Coordinate care with patients and families and provide instructions.
• Communicate care updates to the Case Manager.

What You’ll Do:

• Conducting Start of Care comprehensive assessments and completing OASIS timely
• Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
• Preparing accurate, orderly, and timely documentation
• Coordinating care with patient and provide proper reports to Case Manager
• Provides health care instructions to the patient/family as appropriate per assessment and plan
