Job summary

Advantage Home Health Services in State College, PA is seeking a Home Health Registered Nurse for a full-time role. You will conduct Start of Care assessments, develop and implement care plans, and ensure accurate documentation while coordinating with the Case Manager to ensure optimal patient outcomes.

The ideal candidate holds an active RN license in PA or eligibility to take NCLEX, with experience in home health.

Qualifications

• Active RN license in the state of practice or eligibility to obtain license.
• Experience in home health or Oasis/ OASIS knowledge preferred.
• Strong communication and accurate documentation skills.

Responsibilities

• Start of Care assessments and timely completion of OASIS.
• Draft care plans and reconcile patient medications; ensure orders are accurate.
• Prepare accurate, orderly, and timely documentation.
• Coordinate care with patient and provide reports to the Case Manager.
• Provide health care instructions to patient/family per assessment and plan.

What You’ll Do:
