Job summary

Advantage Home Health Services in Allison Park, PA is seeking a part-time Home Health Registered Nurse to deliver in-home care. This role focuses on comprehensive assessments, accurate documentation, and collaborating with Case Managers to ensure quality patient outcomes.

As a team member, you will develop care plans, reconcile medications, and provide patient and family education in alignment with the plan of care. Active PA RN license or NCLEX eligibility for new grads is required.

Responsibilities

• Conduct Start of Care comprehensive assessments and complete OASIS timely.
• Draft care plans, reconcile patient medications, and ensure orders for care are accurate and complete.
• Prepare accurate, orderly, and timely documentation.
• Coordinate care with patient and provide proper reports to Case Manager.
• Provide health care instructions to the patient/family as appropriate per assessment and plan.
