Job summary

Advantage Home Health Services in Allison Park, PA is seeking a part-time Home Health Registered Nurse to deliver in-home patient care and support our mission of better service, quality and results.

You will perform start-of-care assessments, build care plans, reconcile medications, document thoroughly, and coordinate with the Case Manager to ensure safe, effective care for patients.

Qualifications

• Active RN license in the state of practice.
• New Grad with eligibility to take NCLEX.

Responsibilities

• Start of Care comprehensive assessments and timely OASIS completion.
• Drafting care plans and reconciling patient medications.
• Ensuring orders for care are accurate and complete.
• Preparing accurate, orderly, and timely documentation.
• Coordinating care with patients and providing reports to the Case Manager.

What You'll Do:

• Conducting Start of Care comprehensive assessments and completing OASIS timely
• Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
• Preparing accurate, orderly, and timely documentation
• Coordinating care with patient and provide proper reports to Case Manager
• Provides health care instructions to the patient/family as appropriate per assessment and plan
