Job summary

Advantage Home Health Services in Huntingdon, PA seeks a dedicated Home Health Registered Nurse to join our growing team. The role involves direct patient care in home settings, performing Start of Care assessments and timely OASIS, and drafting care plans under Case Manager supervision.

You will reconcile medications, ensure orders are accurate, document thoroughly, and coordinate care with patients and families to support their health goals.

Qualifications

• Active RN license in the state of practice (or NCLEX eligibility for new grads).
• Ability to perform Start of Care assessments and complete OASIS in a timely manner.

Responsibilities

• Conduct Start of Care comprehensive assessments and complete OASIS timely.
• Draft care plans, reconcile patient medications, and ensure orders for care are accurate and complete.
• Prepare accurate, orderly, and timely documentation.
• Coordinate care with patient and provide proper reports to Case Manager.
• Provides health care instructions to the patient/family as appropriate per assessment and plan.
