Job summary

Advantage Home Health Services in State College, PA is seeking a Home Health Registered Nurse to join our growing team. This full-time role involves delivering high-quality in-home patient care, conducting Start of Care assessments, and coordinating with Case Managers to ensure accurate plans and documentation.

Qualifications

• Active RN license in the state of practice or New Grad with eligibility to take NCLEX.
• Possess in-home care understanding and nursing skills.

Responsibilities

• Conduct Start of Care comprehensive assessments and complete OASIS timely.
• Draft care plans, reconcile patient medications, and ensure orders for care are accurate and complete.
• Prepare accurate, orderly, and timely documentation.
• Coordinate care with patient and provide proper reports to Case Manager.
• Provide health care instructions to the patient/family as appropriate per assessment and plan.
