Addus HomeCare in Kalispell, MT is seeking a dedicated Registered Nurse to provide professional home health care in accordance with the established plan of care and company policies. You will assess, plan, and implement nursing care in patients' homes, while coordinating with families and the health team.

The role requires an active MT RN license, 1+ year clinical experience (ICU/rehab/med-surg preferred), and reliable transportation for home visits.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as an RN in the state of practice.
• At least 1 year of clinical nursing experience in ICU, rehab, or medical-surgical care.
• Home health experience preferred.
• Ability to make home visits and provide nursing care in the home environment.
• Ability to complete documentation timely and legibly.
• Strong communication and teamwork skills.
• Reliable transportation with valid driver's license and insurance.

Responsabilités

• Assess, plan and intervene using the nursing process.
• Observe and report changes in condition and document in the clinical record.
• Administer treatments and medications per plan of care.
• Educate patients and caregivers on self-care techniques.
• Notify Case Manager, physicians, and families of changes.
• Supervise LPNs and Home Health Aides per state regulations.
• Maintain data collection and reporting systems.
• Ensure compliance with Medicare, Medicaid, HIPAA regulations and agency policies.
• Participate in multidisciplinary meetings and on-call scheduling.
• Coordinate care with patient, family and health care team.
