Job summary

Addus Homecare is seeking a licensed Registered Nurse to deliver professional nursing care in the home setting in Great Falls, MT. You will assess patients, implement care plans, and coordinate with the health care team to ensure quality outcomes.

Requirements include an RN license in the practicing state, at least one year of clinical nursing experience (home health preferred), and the ability to travel to patient homes while maintaining thorough documentation and adherence to policies.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as an RN in the state of practice.
• At least 1 year of clinical nursing experience, preferably in home health or related settings.
• Home health experience is preferred.
• Ability to perform home visits and provide nursing care in the home environment.
• Ability to complete documentation accurately, legibly, and timely.
• Strong interpersonal, verbal, and written communication skills.

Responsibilities

• Assess, plan and intervene using the nursing process to meet patient needs.
• Observe and report changes in patient condition and notify supervisor.
• Administer treatments and medications per plan of care and state Nurse Practice Act.
• Teach patients and caregivers self-care techniques.
• Notify Case Manager and physicians of changes in condition.
