Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as an RN in the state of practice.
• At least one year clinical nursing experience, preferably in home health or related settings.
• Ability to make home visits and provide nursing care in the home environment.
• Ability to complete necessary documentation accurately and timely.
• Strong interpersonal, verbal and written communication skills.

Responsibilities

• Assess, plan and intervene using the nursing process to meet the patient's health care needs.
• Observe and report signs or symptoms of changes in condition; document and notify supervisor.
• Administer treatments and medications per the plan of care and state regulations.
• Teach patients/caregivers self-care techniques.
• Notify the Case Manager, physicians, and families of changes in patient condition.
• Perform LPN and Home Health Aide supervisory visits according to state regulations.
• Maintain data collection, recording, and reporting systems for outcomes measurement.
• Communicate risk management issues and report abuse or unsafe practices as required by law.
• Attend multi-disciplinary patient case conferences, staff meetings and in-service education programs.
• Coordinate patient care with patient, family, and health care team members.
• Participate in on-call schedule and after-hours visits as necessary.
• Maintain confidentiality at all times.
• Maintain regular attendance and be flexible to agency needs.
• Follow Medicare, Medicaid, and HIPAA regulations and requirements.
• Performs other duties as assigned.
