Alabama HomeCare of Fort Payne, part of the LHC Group within the Optum family, invites interested RNs to provide and direct nursing care in patients' homes. You will coordinate the total plan of care with physicians and the health care team, ensuring continuity and high-quality care in the home setting.

Responsibilities include assessing patients (OASIS), supervising aides, documenting visits, and communicating with families to support positive outcomes and compliance with state regulations.

Formación

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.

Responsabilidades

• Initiate, develop, implement, and revise the plan of care with the physician and other health care professionals.
• Supervise home health aides and licensed nurses and assign tasks per regulations.
• Provide required supervisory visits and comprehensive nursing evaluations (OASIS).
• Document patient visits per policy and payer requirements; ensure timely communication with the care team.
• Communicate with physicians, patients, and families to ensure quality care and service excellence.
