Job summary

Southeast Alabama HomeCare, a part of LHC Group, is seeking a Registered Nurse to provide and direct nursing care to home health patients as prescribed by physicians and in line with agency policies.

You will coordinate the plan of care with other health professionals, supervise home health aides and RNs, perform assessments (OASIS), document visits accurately, and participate in quality and compliance activities to ensure safe, effective care in the patient’s home.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements, and syncs timely per LHC policy.
• Communicates relevant information timely with physicians, patients, and family to ensure quality care and service.
