Aufgaben

• Develops, implements, and updates patient plans of care with physicians and interdisciplinary team.
• Supervises home health aides and LPN/LVN staff in accordance with regulations.
• Documents patient care per policy and communicates with healthcare team to ensure continuity of care.
• Participates in orientation and performance improvement activities; supports survey readiness and patient safety.
• Demonstrates professionalism and participates in on-call/weekend rotations.
• Ability to work in diverse home settings and with diverse patient populations.
