Job summary

Infirmary HomeCare, part of LHC Group within the Optum family, seeks a Registered Nurse in Home Health to deliver compassionate, skilled care in patients' homes in Mobile, Alabama.

You will coordinate the total plan of care with physicians and the care team, supervise home health aides and nurses, perform OASIS assessments, and ensure documentation and compliance with state and federal regulations. A valid RN license and reliable transportation are required.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification within 90 days (preferred).
• 1+ year of Home Health experience (preferred).
• Ability to work independently and communicate effectively.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and health care professionals.
• Supervises care provided by home health aides and LPN/LVN, provides instruction, and assigns tasks per regulations.
• Provides required supervisory visits.
• Provides high-quality clinical services per plan of care and infection control standards; coordinates with the health care team.
• Completes OASIS assessments and medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements; adheres to LHC timelines.
• Performs initial/comprehensive nursing evaluations; ensures eligibility/medical necessity; sets care focus and plans with physician; documents accurately.
• Communicates timely with staff on patient issues, visits, schedules, orders, OASIS data, and coordination with clinicians.
• Maintains timely communication with physicians, patients, and families to ensure quality care.
• Takes direction from Clinical/Executive Directors; completes tasks on time and participates in training.
• Assists in onboarding new staff and acts as a preceptor.
• Participates in survey readiness and quality improvement to reduce hospitalizations and improve patient safety.
• Participates in on-call and weekend rotations as needed.
• Follows the agency's utilization management model.
• Ability to work with patients of all ages and backgrounds.
