Job summary

Mountaineer Home Care, part of LHC Group and a member of the Optum family, seeks a Registered Nurse in Home Health to deliver and direct nursing care for patients in their homes, in accordance with physician orders and agency policies, and to coordinate the total plan of care with other health professionals.

Primary responsibilities include initiating and revising the plan of care with the physician, supervising home health aides and licensed nurses, performing comprehensive assessments (OASIS),

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits.
• Provides high-quality clinical services within the plan of care and infection control standards, in coordination with the health care team.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements, and syncs timely per agency policy.
• Makes initial and/or comprehensive nursing evaluation visits and ensures patients meet home health eligibility and medical necessity.
• Communicates information timely with physicians, patients, and family to ensure quality care and service excellence.
• Takes direction from Clinical Director and Executive Director professionally and completes assigned tasks timely.
• Participates in survey readiness activities and performance improvement plans, and works to reduce unnecessary hospitalizations.
