Idaho Home Health, part of LHC Group within the Optum family, is seeking an experienced Registered Nurse to deliver and direct nursing care in patients' homes. You will coordinate the plan of care with physicians and healthcare professionals to ensure continuity of patient care and compliance with policies.

Responsibilities include comprehensive assessments (OASIS), supervision of home health aides, and accurate documentation.

Qualifications

• Current and unrestricted RN licensure in Idaho
• Current Driver's License and access to a dependable vehicle or public transportation
• Preferred CPR certification or ability to complete within 90 days of hire
• 1+ years of Home Health experience
• Ability to work independently
• Solid communication, writing, and organizational skills

Responsabilités

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
• Supervises care provided by home health aides and licensed practical nurses, provides instruction, and assigns tasks per state and federal regulations
• Provides supervisory visits and ensures compliance with applicable laws and agency policies
• Performs comprehensive assessments (OASIS), medication reconciliation, and accurate documentation of patient visits
• Communicates with physicians, patients, families, and agency staff to ensure quality care and timely information exchange
• Participates in on-call and weekend rotation as needed
• Adheres to utilization management and performance improvement initiatives
