Job summary

Apex Home Health, a part of LHC Group, is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes, coordinating the plan of care with the physician and other health care professionals.

You will assess patients, supervise home health aides and licensed nurses, ensure documentation accuracy (OASIS), and collaborate with physicians, families and community resources to maintain continuity of care.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to a dependable vehicle.

Responsibilities

• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to state and federal regulations.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements, and syncs timely per policy.
• Communicates relevant information with appropriate agency staff, physicians, patients and families to ensure quality care.
