• Ability to perform Start of Care assessments and document findings.

Responsibilities

• Conduct Start of Care assessments and timely OASIS completion.
• Draft and reconcile care plans; verify orders for care.
• Prepare accurate, organized documentation and reports.
• Coordinate care with patients and provide reports to the Case Manager.
• Deliver health care instructions to patients and families as appropriate.

What You'll Do:

• Conducting Start of Care comprehensive assessments and completing OASIS timely.
• Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete.
• Preparing accurate, orderly, and timely documentation.
• Coordinating care with patient and provide proper reports to Case Manager.
• Provides health care instructions to the patient/family as appropriate per assessment and plan.
