Job summary

Millennium Physician Group is seeking a licensed Physical Therapist Assistant to join the Home Health Team in Lee County, Florida. You will evaluate patients, develop individualized plans, and guide therapeutic interventions to improve mobility, strength, balance, and independence in patients’ homes.

Responsibilities include patient education, collaboration with the care team, and meticulous documentation. Must have a Florida PTA license and at least 6 months of relevant experience in hospital,

Qualifications

Current and valid Physical Therapist Assistant (PTA) license in Florida.
Minimum of 6 months experience in hospital, skilled nursing facility, or rehab.

Responsibilities

Provide skilled therapy services under the supervision of a Physical Therapist.
Implement treatment plans with exercises, gait training, and assistive device training.
Educate patients and families on techniques to improve function and independence.
Collaborate with Physical Therapists, occupational therapists, and other providers to optimize outcomes.
Maintain accurate and timely documentation of patient progress, plans, and interventions.
Adhere to infection control guidelines to protect patients and staff.
