Qualifications

• Florida Physical Therapist license required.
• Minimum 1 year clinical PT experience, home health preferred.
• Strong clinical, communication and interpersonal skills; patient-centered approach.
• Professionalism, reliability and commitment to high-quality care.
• Reliable transportation to visit patients at home.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Provide manual therapy, therapeutic exercises, and gait training to improve mobility and strength.
• Educate patients and families on home exercise programs and injury prevention.
• Collaborate with physicians, nurses, and therapists to create comprehensive care plans.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries.
• Ensure compliance with HIPAA and Medicare regulations.
