Job summary

Millennium Physician Group is seeking a Physical Therapy Assistant (PTA) for its Home Health team in Lee County, Florida. The role focuses on evaluating patients at home, creating individualized plans, and delivering interventions to improve mobility and independence.

You will collaborate with a multidisciplinary team, educate patients and families, and maintain compliant clinical documentation while adhering to infection control standards.

Qualifications

• Current and valid Florida PTA license.
• Minimum 6 months hospital, skilled nursing facility, or rehab experience.
• Excellent communication, interpersonal, and critical thinking skills.
• Reliable transportation to visit patients at their homes.

Responsibilities

• Provide skilled therapy services under the supervision of a Physical Therapist.
• Implement treatment plans, including therapeutic exercises, gait training, and assistive device training.
• Educate patients and families on rehabilitation and prevention strategies.
• Collaborate with Physical Therapists, occupational therapists, and other healthcare professionals to ensure optimal outcomes.
• Maintain accurate and timely documentation of patient progress, treatment plans, and interventions.
• Adhere to infection control guidelines to protect patients and staff.
