Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Associate degree in Physical Therapist Assistant studies or equivalent.
• Current, unrestricted Ohio PTA license.
• CPR certification, preferably BLS.
• Valid driver's license and reliable transportation.
• Automobile insurance meeting applicable legal and agency requirements.

Responsabilités

• Provide skilled PT interventions per plan of care.
• Implement exercises, gait training, balance, transfers, and mobility interventions.
• Instruct patients and caregivers in home exercise programs and safe mobility techniques.
• Monitor responses and adjust interventions within plan of care.
• Travel to patients' homes within service area.
• Maintain timely documentation and HIPAA/compliance.
• Communicate changes in patient condition to supervising PT.
• Promote fall prevention and home safety education.
• Collaborate with interdisciplinary team to ensure continuity of care.
