Job summary

CenterWell Home Health seeks a licensed Physical Therapist Assistant to administer the physical therapy plan at patients' homes under the supervision of the Physical Therapist. Qualifications

• Current state license to practice Physical Therapy is required.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Comprehensive knowledge of geriatric physical, emotional, social, and biological changes.

Responsibilities

• Administer the physical therapy treatment plan to patients, onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supplies, staff or client changes.
• Apply infection control and universal precautions in patient care.
