Job summary

PruittHealth is seeking a Physical Therapist (PT) for Home Health services in North Carolina. You will evaluate function, provide interventions in patients’ homes, and collaborate with physicians and families to achieve optimal outcomes.

New pay-per-point model offers top pay in the industry, with mileage reimbursement and opportunities for career growth. Prior home health experience is preferred; licensure must be active in the state.

Qualifications

• Must have a valid professional license in state of practice.
• Graduate of an accredited Physical Therapy Program.
• If foreign-trained, the program must be deemed substantially equivalent to a Physical Therapist entry level education in the U.S. by a credentials evaluation organization approved by the APTA.

Responsibilities

• Evaluate the patient's level of function and skilled therapy needs.
• Treat the patient to relieve pain, develop or restore function, and obtain maximum outcomes according to the established plan of care.
• Advise and consult the family and interdisciplinary team to promote optimal functional outcomes.
• Assesses, monitors, documents and reports progress of patient health and condition using required documents and timelines.
• Helps maintain continuity of care by coordinating with the team and physician to manage the care plan.
• Demonstrates knowledge of home care and competency for patient care skills.
• Conducts field visits to evaluate and initiate care and monitor outcomes.
• Completes documentation timely and accurately at the point of care.
