Job summary

CenterWell Home Health is seeking a Home Health Physical Therapy Assistant to work in patients’ homes under the supervision of a Physical Therapist. You will implement the therapy plan, perform therapeutic exercises, document care, and participate in department in-services to support patient independence and quality of life.

The role requires a current Physical Therapy license, a valid driver's license, CPR certification, and English fluency, with preferred 1 year of PTA experience.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of changes in the geriatric population.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, lack of supplies, or staff or client changes.
• Apply infection control and universal precautions in performing patient care activities.
