Job summary

CenterWell Home Health seeks a licensed Physical Therapy Assistant to provide in-home PT services under the supervision of a Physical Therapist. You will work onsite in patients' homes, delivering prescribed treatments and documenting progress.

Requirements include an active PT license, valid driver's license with insurance, CPR certification, and strong teamwork skills. Experience with geriatric patients and fluency in English are preferred.

Qualifications

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of geriatric population changes.
Read, write, and speak fluent English.

Responsibilities

Administer physical therapy treatment as prescribed by the PT.
Deliver non-complex manual therapy, massage, traction, heat/cold, electrical modalities.
Document client records and submit weekly documentation.
Attend educational meetings and coordinate department in-services.
Notify PT or Branch Director of equipment failures or supply issues.
Apply infection control and universal precautions in patient care.
