Job summary

3HC Home Health and Hospice seeks a Physical Therapist Assistant to provide prescribed therapies in patient homes and clinical settings in Pollocksville, NC. The role supports licensed therapists, documents visits, and ensures care plans are followed with quality and compassion.

Travel to other offices may be required to meet patient needs. Must be a graduate PTA, licensed in North Carolina, with CPR certification and strong patient care skills aimed at restoring function and independence.

Qualifications

• Graduate of an approved Physical Therapy Assistant curriculum.
• One-year of clinical experience preferred.
• Licensed Physical Therapist Assistant currently licensed to practice physical therapy in North Carolina.
• Maintain current CPR certification (BCLS).

Responsibilities

• Assists the licensed physical therapist in treating patients to relieve pain and restore function.
• Delivers prescribed physical therapy treatments and documents each visit.
• Instructs patients and families in use of assistive devices and home programs.
• Maintains timely clinical and administrative records and manages chart documentation.
• Communicates patient status and changes to the supervisor and therapist.
• Travel to other offices as needed to meet patient demand.
