Job summary

CenterWell Home Health is seeking a Licensed Physical Therapy Assistant to work in patients' homes in the Charlotte, NC area. You will implement the treatment plan created by the Physical Therapist, perform therapeutic exercises, document care, and coordinate with the clinical team to support patient independence and safety.

Ideal candidates hold a current state license, CPR certification, a valid driver's license with reliable transportation, and the ability to work collaboratively with a care

Qualifications

Current state license to practice Physical Therapy.
Valid driver's license and reliable transportation.
CPR certification required.
Experience collaborating with a healthcare team.
Geriatric population knowledge.
English reading/writing/speaking proficiency.

Responsibilities

Administer PT treatment plans in patients' homes.
Execute non-complex therapeutic exercises and modalities.
Document client records and submit reports weekly.
Attend educational meetings and coordinate in-services.
Report equipment issues or staff/client changes.
Apply infection control and safety precautions.
