Job summary

CenterWell Home Health in Charlotte, NC is seeking a licensed Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist. You will deliver the prescribed plan, perform therapeutic exercises, and help patients regain strength and independence.

This role requires a current state PT license, valid driver’s license, CPR certification, and the ability to work collaboratively with a care team.

Qualifications

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of geriatric changes.
Read, write, and speak fluent English.

Responsibilities

Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify Physical Therapist or Branch Director of equipment failures or staff changes.
Apply concepts of infection control and universal precautions in patient care.
