Qualifications

Associate degree in Physical Therapy from an American Physical Therapy Association program.
Current state license to practice Physical Therapy.
Valid driver's license, auto insurance and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Knowledge of geriatric physical, emotional and social changes.
Read, write, and speak fluent English.

Responsibilities

Administer the physical therapy treatment plan to patients, working onsite in their homes as developed by the PT.
Administer non-complex active and passive manual therapies, massages, traction, heat/cold, and electrical modalities.
Document client records and submit clinical documentation to the office weekly.
Attend educational meetings and coordinate department in-services.
Notify PT or Branch Director of equipment issues, supply shortages, or staff/client changes.
Apply infection control and universal precautions in patient care activities.
