Qualifications

• Active Texas PTA license.
• Current CPR certification.
• Professional liability insurance.
• Experience in home health or rehabilitation settings preferred.
• Proven ability to collaborate within a multidisciplinary care team.
• Demonstrated clinical experience as a PTA.
• Strong interpersonal skills and adaptability in patient care.
• Solid understanding of current treatment techniques and therapy practices.

Responsibilities

• Deliver physical therapy services in the home setting under the supervision of a licensed Physical Therapist and per physician plan of care.
• Identify patient goals and implement interventions to meet functional and mobility needs.
• Provide evidence-based, cost-effective treatments to improve movement and independence.
• Support alternatives to surgery and reduce reliance on medications through therapy.
• Develop and carry out individualized care plans using proven techniques.
• Create wellness and fitness programs tailored to each patient’s condition and goals.
• Educate and motivate patients to actively participate in rehabilitation and recovery.
• Collaborate with other healthcare professionals to ensure high-quality care.
• Monitor patient progress and document treatment outcomes and records.
• Perform additional duties as assigned by the supervising therapist or clinical manager.
