CenterWell Home Health is seeking a Home Health Physical Therapist Assistant in Tennessee to work under the supervision of the Clinical Manager. You will deliver in-home physical therapy services, focusing on functional improvement and independence for geriatric patients.

Qualifications

• Current state license to practice Physical Therapy (PT)
• Valid driver's license, auto insurance and reliable transportation
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Comprehensive knowledge of geriatric physical, emotional, social and biological changes
• Read, write, and speak fluent English

Responsibilities

• Administer the physical therapy treatment plan to patients onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex manual therapeutic exercises, massage, traction, heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation timely.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply issues, or staff or client changes.
• Apply infection control and universal precautions in patient care.
• Utilize skills to make an impact.
