CenterWell Home Health seeks a Home Health Physical Therapist Assistant to help patients regain strength and independence in their homes under the Clinical Manager's guidance. You will implement treatment plans and coordinate care with a team to improve mobility and quality of life.

You will document care, participate in departmental trainings, and ensure infection control practices. A valid PT license, CPR, and reliable transportation are required to serve geriatric patients in the community.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatrics and related physical, emotional, social, and biological changes.

Responsabilités

• Administer the physical therapy treatment plan to patients, working onsite in their homes.
• Administer non-complex active and passive manual therapies and modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify supervisor of equipment issues, supply shortages, or client changes.
• Apply infection control and universal precautions in patient care.
