PHC Home Health is seeking a dedicated Physical Therapist Assistant to work under a licensed Physical Therapist and provide in-home care in North Charlotte, Lake Norman and surrounding areas. You will help patients regain strength, manage pain, and support treatment plans while documenting progress and coordinating with the healthcare team.

Requirements include an Associate's degree in Physical Therapy Assistant, current state license, CPR, and at least 1 year of home health experience; a valid

• Associate's degree in Physical Therapy Assistant program.
• Current state license as a Physical Therapist Assistant.
• Minimum of 1 year of experience in home health or related setting.
• CPR Certification.
• Excellent communication and interpersonal skills.

Responsabilités

• Assist the Physical Therapist in evaluating patients and following treatment plans
• Provide physical therapy services to patients in their homes
• Help patients regain their strength and mobility, manage pain, and prevent further injury or disability
• Document patient progress and communicate with other healthcare professionals as needed
• Provide education to patients and their families about their condition and treatment plan
• Adhere to all state and federal regulations and guidelines for physical therapy services
