CenterWell Home Health, a Humana Inc. company, is seeking a Home Health Physical Therapist Assistant to support patients in their homes under the Clinical Manager. You will administer the therapy plan, document care, and work with the Physical Therapist to drive functional improvements.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric physical, emotional, social and biological changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, massage, traction, heat, light, cold, water and electrical modalities.
• Document client records and submit clinical documentation to the office weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures or staffing changes.
• Apply infection control and universal precautions in patient care.
