CenterWell Home Health, a Humana Inc. company, seeks a Home Health Physical Therapist Assistant to deliver PT at patients’ homes under the supervision of a Physical Therapist. You’ll help patients regain strength and independence through personalized care and functional improvements.

You will administer exercises, document records, participate in in-services, and ensure infection control. Requires state PT license, valid driver’s license, CPR, and team collaboration, with a focus on geriatric

Qualifications

Current state license to practice Physical Therapy.
Valid driver's license and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of geriatric changes.

Responsibilities

Administer the physical therapy treatment plan to patients in their homes, as developed by the Physical Therapist.
Perform non-complex manual therapy, modalities, and related treatments as directed.
Document client records and submit clinical documentation weekly.
Attend educational meetings and department in-services.
Notify PT or Branch Director of equipment or staff changes.
Apply infection control and universal precautions in patient care.
