Job summary

CenterWell Home Health, a Humana company, is seeking a Home Health Physical Therapist Assistant. You will deliver personalized PT care in patients' homes, implementing the plan developed by the Physical Therapist to help patients regain strength, mobility and independence.

Reporting to the Clinical Manager, you will document visits, collaborate with a care team, maintain infection control standards and drive outcomes. Requires state PT license, CPR, driver’s license and reliable transportation.

Qualifications

• Current state license to practice Physical Therapy or PTA license
• Current CPR certification
• Valid driver's license and reliable transportation
• Experience collaborating with a healthcare team
• Comprehensive knowledge of geriatric physical changes
• Read, write, and speak fluent English

Responsibilities

• Administer PT treatment plan in patients' homes as developed by PT
• Administer non-complex manual therapy and modalities
• Document client records and submit weekly clinical documentation
• Attend educational meetings and coordinate department in-services
• Notify PT or Branch Director of equipment/supply issues
• Apply infection control and universal precautions in patient care
• Contribute to patient improvement and independence
