Job summary

CenterWell Home Health, part of Humana, is seeking a Home Health Physical Therapist Assistant to provide in-home therapy, working with a clinical team to help geriatric patients regain function and independence. You will deliver PT plans, document progress, and participate in departmental education while ensuring safety and infection control.

A valid state license and CPR are required. Candidates must have reliable transportation and possess English fluency.

Qualifications

• Must hold a current state license to practice Physical Therapy.
• Valid CPR certification is required.
• Fluent English in reading, writing and speaking.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes, as developed by the Physical Therapist.
• Perform non-complex active and passive manual therapies and modalities as directed.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify supervisor of equipment failures or staff/client changes.
• Apply infection control principles in care activities.
