Job summary

PruittHealth Home Health is seeking a dedicated Physical Therapy Assistant (PTA) to provide home-based physical therapy services under the supervision of a licensed physical therapist. You will assess patients in their homes, implement the plan of care, document progress, and collaborate with the care team to achieve patient goals.

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• If foreign-trained, the program must be deemed substantially equivalent to a Physical Therapist Assistant entry level education in the U.S. by a credentials evaluation organization approved by the APTA.

Responsibilities

• Provide physical therapy treatments to home health patients in their residence under the supervision of a physical therapist.
• Assist with development and implementation of the plan of care within defined goals.
• Collaborate with the interdisciplinary team to manage and integrate patient care plans.
• Complete timely documentation at the point of care according to standards.
• Assist in assessing physical disability and function levels of patients.
• Maintain licenses, certifications, and records to meet regulations.
• Participate in surveys and required reporting.
