Amedisys Home Health is seeking a dedicated Physical Therapist Assistant to join our home health team in LaFayette, GA. This full-time, day-shift position involves hands-on patient care at home under supervision of a licensed PT, delivering education and interventions per plan of care.

You will observe patient responses, document progress, and coordinate follow-up with the care team to support healing in the patient's home environment.

Formación

• Current license to practice as a PTA in the assigned state.
• Graduate from an accredited PTA program (CAPTE).
• One year PTA experience in home health preferred.
• If less than one year, mentorship/residency is required.
• Current CPR certification.

Responsabilidades

• Provide PT services under supervision of a PT per plan of care.
• Observe and report patient responses and changes to clinicians.
• Promote wellness and prevention.
• Document supervisory activities in the medical record.
• Monitor and document patient responses to PT intervention.
• Participate in care conferences and quality improvement activities.
• Maintain patient confidentiality.
• Coordinate follow-up care and referrals with other agency providers.
• Perform other duties as assigned.

Conocimientos
